Children's Health Home Care Management
Health Home Care Management is a free, voluntary, service for people who qualify for Medicaid and have certain conditions and needs. We offer a comprehensive team approach for our clients and their family to address physical, behavioral/mental, and social needs using a collaborative team approach.

Who qualifies for services?
If you are enrolled in Medicaid and have a child younger than 21, your child must have at least one or more of the following:

A least two chronic health conditions

A diagnosed Severe Mental Illness (SMI), including a Severe Emotional Disturbance (SED).

Have experienced trauma and is at risk for another chronic condition.

Our highly skilled and effective Care Managers work with all your current care providers to help you create a single plan of care that builds on your family’s strengths and addresses your individual concerns. This may also include school-related concerns. When additional services or resources are needed our Care Managers can assist you on getting those as well.
A Care Manager will provide:

Comprehensive Care Management

Care Coordination and Health Promotion

Comprehensive Transitional Care

Individual and Family Support Referrals to Community and Social Support Services

Assistance managing all types of care, including: medical, counseling, and social services
In order to achieve the best results, we encourage families within our program to complete one face-to-face visit with member of our team each month.
